You can request a copy of Vital Statistic records in the County Clerk's office at the Old Lee County Courthouse located on the second floor.

You may also send a request letter with name of child, date of birth, mother's maiden name, father's name, and a fee of $12.00 with a self-addressed envelope to the County Clerk's office.

Fee Schedule:

	Certified Copy (Birth & Marriage)	$12.00
	- each additional copy	$2.00
	Death Certified Copy	$14.00
	- each additional copy	$4.00

All vital records must be a Certified copy
