You can request a copy of Vital Statistic records in the County Clerk's office at the Old Lee County Courthouse located on the second floor.

You may also send a request letter with name of child, date of birth, mother's maiden name, father's name, and a fee of $20.00 with a self-addressed envelope to the County Clerk's office.

	Death Certified Copy	$30.00
	- each additional copy	$10.00
	Issue a Marriage License	$35.00
	File an Assumed Name	$15.00
